Diabetes Recognition Program

  Posted Jan 10, 2013

We would like to remind you to submit your Diabetes Recognition Program application by the end of 2012.

The Diabetes Recognition Program is an incentive program run by the National Committee for Quality Assurance (NCQA), that recognizes physicians who have provided exceptional care to their patients with diabetes. Physicians may earn NCQA Recognition by supplying patient data from 25 eligible patients with diabetes in a 12-month period. The standard of care given to these patients is then evaluated according to a scoring system that uses 11 measures to assign points. If a physician earns 75 points or more, he/she will achieve NCQA Recognition and receive a payment of $2,000. Five percent of all physicians are randomly selected for audit and will be required to submit documentation of reported care.

Physicians do not have to be primary care physicians (PCPs) to participate in this program. Eligible patients must be between 18 and 75 years old, and have been under the care of the applicant for at least one year before their most recent visit date. They may belong to any insurance, or be cash patients.

If you would to participate in the Diabetes Recognition Program, please see the information packet here, and contact Dana Zhu at dzhu@ccaco.org or 212-965-0222.
