The Chinese Community Accountable Care Organization (CCACO) would like to invite you to a press conference on Wednesday, September 14, 2016 to announce that it has earned $4.3 million in shared savings through the Medicare Shared Savings Program. CCACO ranks second among all 35 New York State accountable care organizations (ACOs), and in the top 5% of all ACOs nationwide, for percentage of expenditures saved in 2015.

We are pleased to announce that we have successfully completed the 2015 CMS Quality Measures Reporting.
Thanks to everyone’s hard work, including the CCACO participating providers and their office staff, our administrative team and our ACO IT platform, MDLand, CCACO has completely reported 100% of the required quality measures for 2015, prior to the March 11 deadline.
Congratulations to all! Congratulations to all! The reporting period was open for 8 weeks, from January 18, 2016 to March 11, 2016, during which CCACO reported data on over 3,000 Medicare fee-for-service beneficiaries. As a result of our successful reporting, CCACO will be eligible for shared savings, and eligible CCACO providers will avoid the 2015 Physician Quality Reporting System (PQRS) penalties, and be eligible to receive quality incentive payments through the Value-Based Modifier initiative..
CCACO physicians and their staff, as well as CCACO's administrative staff, in conjunction with MDLand, worked tirelessly on this project during the reporting period, demonstrating the great teamwork and commitment of this organization. We look forward to continuing to work together to achieve future successes and to improving patient care within the Chinese community.
We would like to sincerely thank our membership for their cooperation and dedication in helping CCACO to successfully complete 100% of the CMS reporting requirement.
CCACO presented an overview of the CMS Quality Measures Reporting process, including its timeline (January 27, 2014 – March 27, 2014) and the data required from CCACO members. Attendees were introduced to the 15 data modules and were taught to use the application that would allow them to submit their data. CCACO informed the attendees that they would receive five dollars for each completed patient, and ten dollars for each patient completed on or before March 27, 2014.
In addition, CCACO presented data on the predicted shared savings and encouraged its members to purchase additional shares.

On Saturday, September 21, 2013, CCACO held a very successful General Shareholder Meeting and Election at the Sheraton Hotel in Flushing. Thank you to everyone who attended the event and helped to make the meeting a great success.

We are now accepting a second round of applications for the Diabetes Recognition Program. Applicants must manage the care of diabetic patients between the ages of 18 and 75, and submit data on a sample of 25 diabetic patients. Successful applicants will be recognized by the National Committee for Quality Assurance, and receive an award of $2,000.

It is with great pleasure that we announce that 51 CCACO physicians have earned National Committee for Quality Assurance Recognition in the Diabetes Recognition Program (DRP). The DRP recognizes physicians who have provided outstanding care to their diabetic patients. Thanks to a grant CCACO received from the NYS Health Foundation, each physician will receive an award of $2,000 in recognition of the high standard of care they provided to their diabetic patients. Physicians will also be featured in the Clinician Recognition Directory on the NCQA website.

The reporting period was open for 8 weeks, from February 15, 2013 to April 15, 2013, during which CCACO reported data on over 3,000 Medicare fee-for-service beneficiaries. As a result of our successful reporting, CCACO will be eligible for shared savings, and eligible CCACO providers will qualify for 2012 Physician Quality Reporting System (PQRS) payments.
As a thank you, we will be sending a token of our appreciation to each member who aided us in completing the 2012 CMS Quality Measures Reporting.

We invite you to participate in the 2013 CCACO Board of Directors Call for Nominations. The Nomination Committee is now recruiting and evaluating candidates to serve on the Board of Directors. This is a very important task. Please help us by nominating your colleagues and friends who are strong leaders and eager to serve on CCACO's Board of Directors.
During the September 21, 2013 General Shareholder Meeting, the CCACO general membership will vote for and elect four (4) colleagues - 1 PCP and 3 Specialists - to serve on the CCACO Board of Directors. The criteria to be a Board of Director Candidate are:
• | Nomination and endorsement by a minimum of five (5) CCACO members |
• | Nominees must be a CCACO Member and a physician in good standing |
• | Nominees must agree to serve a 2 year term |
• | Nominees must not serve as a Board member of another ACO or have a conflict of interest with CCACO |
• | Nominees must agree to attend minimum of 75% of board meetings |
• | Elected Candidates must agree to serve on CCACO committees as required |
The time is here to report our CMS quality measures. The reporting period is only open for 8 weeks - 2/15/13 - 4/15/13. We need your help and support to collect the required data, which is crucial to receiving our shared savings.
If Medicare Fee-for-Service patients from your practice have been chosen by CMS for reporting, our team will contact your office in the next week to meet with you. Please identify a contact person dedicated to complete this task so our team can show them what to do.

We have been working on notifying all 12,000 of our Medicare Fee-for-Service beneficiaries. We would like to thank those who have already begun to notify their Medicare Fee-for-Service patients, and urge those who have not yet started to begin as soon as possible. We would like to remind you to submit your Diabetes Recognition Program application by the end of 2012.
The Diabetes Recognition Program is an incentive program run by the National Committee for Quality Assurance (NCQA), that recognizes physicians who have provided exceptional care to their patients with diabetes. Physicians may earn NCQA Recognition by supplying patient data from 25 eligible patients with diabetes in a 12-month period. The standard of care given to these patients is then evaluated according to a scoring system that uses 11 measures to assign points. If a physician earns 75 points or more, he/she will achieve NCQA Recognition and receive a payment of $2,000. Five percent of all physicians are randomly selected for audit and will be required to submit documentation of reported care.
Physicians do not have to be primary care physicians (PCPs) to participate in this program. Eligible patients must be between 18 and 75 years old, and have been under the care of the applicant for at least one year before their most recent visit date. They may belong to any insurance, or be cash patients.

CCACO members are invited to purchase additional shares. Each additional share may be purchased for $1,000 (i.e. $1,000 = 1 share). Founder shareholders may own up to 24 shares. Non-founder shareholders may own up to 20 shares. Please review the Guide to Purchasing Additional Shares to assist you in determining how many additional shares you may purchase.
